Information for service users, family and referring agencies

  • FAMILY SUPPORT SESSIONS - To help families access a broad array of support and services, including formal and informal support whilst working with community services to promote the well-being of families.
  • FINANCIAL CAPABILITY SESSIONS (one-to-one or group sessions) - This service gives families/individuals the power and confidence to make the most of their money and to improve their lives, we also aim to empower families/individuals to give them the ability to manage their money well.
  • FINANCIAL HEALTH CHECKS - A detailed look into the families/individuals income and expenditure and to highlight any areas of concern, this will also help to determine if there is any financial support that may be required, such as any benefits or debt support.
  • BENEFITS ADVICE AND INFORMATION - This service will help a family/individual look at all avenues of any benefit entitlement that they may be eligible for we will inform them of any eligible entitlements and advise them accordingly.
  • UNDERTAKING SPECIALIST CASEWORK - This service we conduct and represent in benefit appeals, benefit tribunals, employment tribunals. We also conduct specialist support services such as benefit applications of which we support the family/individual with from start to finish.
  • HEALTH CHECKS (in accordance with current government guidelines, policies and procedures) - This is similar to an NHS Health Check which is defined as 'NHS Health checks are designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia'. At Tomorrow's H.O.P.E We look for all these signs however we offer a more in-depth check and look for other conditions, upon completion a personalised health care booklet will be sent to the individual with the findings along with a personalised action plan with any further advice/information or support. (Unlike other services that offer health checks this service is open to all ages)
  • SOCIAL PRESCRIBING - We work within a community referral programme where we can refer families/individuals to a range of local, non-clinical services with the aim to support those to take a greater control of their own health.
  • COMMUNITY ENGAGEMENT SERVICES - Linking an individual/family to their community including social support linking with community services for an holistic service.
  • PROMOTING HEALTHY LIFESTYLE CHOICES (in accordance with the NHS and Public Health England)
  • HEALTH AND SOCIAL CARE SERVICES - This service looks at helping families/individuals maintain independence whilst liaising with community services along with local authorities to help build a more unique and bespoke care package with the correct support and services based on what matters most to them.

The key to all the services we provide is that unlike many other support services or organisations we are not restricted on the length of time we spend with a family or individual and we look at everything surrounding the family/individual which incorporates the holistic view.

  • Our holistic aim is 'to provide support that looks at the whole person, this includes mental health, physical health, emotional, social and spiritual wellbeing, we focus on a person's wellness and not just their illness or condition.'
