Information for service users, family and referring agencies

  • Family Support Sessions - providing families with a wide range of support and services to promote their well-being. We work in partnership with community services to ensure families have access to both formal and informal support.
  • Financial Capability Sessions - offering families and individuals the opportunity to gain power and confidence in managing their finances, while also equipping them with the tools and knowledge to improve their lives. We strive to empower families and individuals to take control of their finances and make the most of their money.
  • Financial health checks - providing a comprehensive review into an individual's or family's income and expenses, helping to identify any potential financial issues and determine if any additional support, such as benefits or debt advice, is necessary.
  • Benefits Advice and Information Service - helping families and individuals explore all potential avenues of benefit eligibility. We will provide them with comprehensive information about their eligibility and offer our expert advice to ensure they receive the entitlements they are entitled to.
  • Specialist Casework Services - including benefit appeals, benefit tribunals, and employment tribunals. We also provide comprehensive support services for benefit applications, from initial application to final resolution. Our team of experts is dedicated to providing the best possible outcome for our clients.
  • Health Checks - At Tomorrow's H.O.P.E, we provide comprehensive health checks in accordance with current government guidelines, policies, and procedures. Similar to an NHS Health Check, we look for signs of stroke, kidney disease, heart disease, type 2 diabetes, and dementia. However, we go further and look for other conditions. After the check, we will provide a personalised health care booklet with the findings, along with a personalised action plan containing any additional advice, information, or support. Unlike other services, our health checks are available for people of all ages.
  • Social Prescribing - we work within a community referral program designed to empower individuals and families to take control of their health. We refer people to a range of local, non-clinical services that provide support and guidance.
  • Comprehensive Community Engagement Services - helping individuals and families establish meaningful connections with their local community. Our services include social support and linkage to community-based services to ensure a holistic approach to meeting the needs of our clients.
  • Promoting Healthy Lifestyle Choices - we are committed to promoting healthy lifestyle choices that are in line with NHS and Public Health England guidelines. We strive to create an environment that encourages and supports healthy habits, such as exercising regularly, eating a balanced diet, and getting enough sleep. We hope to empower our clients to make positive lifestyle choices that will support their long-term health and wellbeing.
  • Health and Social Care Services - Our service is dedicated to empowering families and individuals to maintain their autonomy and independence, while providing tailored support and services based on their individual needs. We collaborate with community services and local authorities to develop customised and comprehensive care packages, and we strive to ensure that all care packages are optimised for each individual, emphasising what matters most to them, to ensure the best possible care.

The cornerstone of all our services is that, unlike many other support services or organisations, we are not limited in the amount of time we spend with a family or individual. We consider all aspects of their life and take a holistic approach.
